PTSD is a terrifying, debilitating, and often isolating condition that is little understood among the population at large. In this blog, I would like to examine what is PTSD, why it is so often misunderstood and misdiagnosed, and what can be done to help those that suffer from it. I have used statistics and examples from 09/11 because most people felt some sort of trauma from just having to live through that particular time. Understanding how traumatic it was for those that were actually present can help us be more sensitive to those that suffer from trauma in general.

One method for assessing the psychological effects of 09/11 is by examining the following: 9/11 is still having a psychological impact on people who were there at or near ground zero eleven years later. By conservative estimates, at least 10,000 police officers, firemen, and civilians exposed to the terrorist attacks have suffered at some point or another from post-traumatic stress disorder, commonly known as PTSD. A conservative estimate calculates that 4,000 of these people still suffer from the condition.

What is PTSD?

The 09/11 survivors often report classic PTSD symptoms to this day, such difficulty sleeping and concentrating, with sleep being disturbed by very lucid, terrifying dreams. They ruminate over the disaster, as if it were a tape inside their head playing on a loop that they cannot turn off. They have what is called a ?startle response? in which they jump or startle to loud noises, and are generally hyperalert to their surroundings. They try, usually without success, to avoid thinking about the traumatic event; unfortunately, even the smallest things can be triggers, reminding constantly them of the trauma.

PTSD also has negative consequences on family life and relationships as sufferers feel like they are in a ?fog? and disconnect with loved ones. Loved ones often hear incredibly frightening thoughts expressed from the sufferers such as the idea that they are going to die soon, or that they do not deserve to live. Survivor?s guilt is common. Loved ones of PTSD sufferers are also frequently disturbed by the dark, negative, and frightening lens through which they view the world; they think bad things happen to good people and are pessimistic about not only their own future, but about the future of all of humanity.

Why PTSD Is So Misunderstood and Misdiagnosed

PTSD, which was first conceptualized in the late 1970?s after experts studied the psychological affects of war on Vietnam veterans, is often misunderstood and the people that suffer from the disorder often do not get the help that they need. Since PTSD symptoms sometimes have a delayed onset, suffers think that they are suffering from simple insomnia, stress or burn-out. With the symptoms occurring months or even years away from the time the trauma took place, a causal relationship between the event and their psychological stress is often missed.

This is compounded by the fact that PTSD is the only psychological condition that is brought on clearly by an ?event?, and the event must be experienced as traumatic. What is traumatic for one person may not be for another; individuals have different levels of resiliency and coping mechanisms, so not everyone is going to suffer from PTSD even if the event is something most people would experience as psychologically injurious. Furthermore, some people are so vulnerable to traumatic loss that even something like a fender-bender can trigger PTSD symptoms for these individuals. Some people, including loved ones, who they themselves are not sensitive to trauma, are often dumbfounded by such sensitivity. Since they do not experience such events as traumatic themselves, they can get exasperated with the sufferer?s morbid preoccupation and emotional debilitation. It can be incredibly confusing and frustrating for the loved ones since the sufferer never wants to talk about ?the event? with them in any meaningful, productive way, but then seem to be able to think about little else since.

PTSD is cumulative, meaning that successive traumatic events eventually catch up with a person psychologically. Sometimes a person has been through several traumatic events and seems able to brush them off quickly. But if there is emotional scar tissue that has been unprocessed then something apparently non-traumatic can flip that person into full PTSD symptomotology. For example, Vietnam veterans have been known not to suffer from PTSD after coming back from duty can sometimes be flipped into the condition by an illness or a natural death of a friend. Individuals that have a history of abuse, neglect, or childhood trauma are more prone to PTSD, and to suffer from PTSD when a traumatic event is less serious.

What Helps

There are a number of treatment options now available for those suffering from PTSD, and the results are often quite positive. I find that psychodynamic psychotherapy is most effective in treating PTSD. Supportive couples counseling can be very beneficial as well, especially if the spouse is having difficulty understanding what their partner is experiencing, and the marriage is suffering. In more treatment resistant cases, a combination of psychiatric medication and psychotherapy is the best approach, but this method is not for everyone and depends on the severity of the symptoms and whether the patient is open to medication management.

Legal Zoom National Survey Shows Increasing Number of Americans ...

LegalZoom.com, the nation?s number one online legal document preparation service, today announced the results of its third national survey on Living Wills, a legal document that allows individuals to decide whether or not to be kept on life support if one is permanently unconscious or in a coma. Conducted by InsightExpress, the survey on Living Wills sampled 1000 adults 18+ (margin of error +/- 3.09 percent) nationwide.

According to the Legal Zoom 2007 survey results, 25.2 percent of respondents had prepared a Living Will, up from 22.1 percent in 2005 and 20.7 percent in 2004.

The 2007 Legal Zoom survey also showed an increase in the number of people who noted the importance of preparing a Living Will: 46.1 percent in 2007 compared to 42.3 percent in 2005. Likewise, those that indicated that having a Living Will was ?not important at all? declined from 23.7 percent in 2005 to 21.4 percent in 2007. There was also a 27 percent decrease in the number of people who ?never even thought about creating a Living Will?: 32.8 percent this year versus 45.2 percent in 2005.

?It?s great that more Americans are making Living Wills and that more people are aware of the importance of this legal document,? noted attorney Robert Shapiro, co-founder of LegalZoom.com. ?The value of preparing a Living Will cannot be overstated, particularly in preventing family and legal disputes if a patient cannot convey his or her wishes regarding life support. Unfortunately, three out of four Americans still haven?t prepared a Living Will.?

Among respondents who considered making a Living Will but still haven?t, the main reasons for their lack of action were ?don?t have the time? (13.1 percent), ?price? (12 percent) and ?trust their spouse with their medical decisions? (17.5 percent).

When asked what prompted respondents to consider preparing a Living Will, more than one out of every five (19.6 percent) cited their own illnesses or an ill friend or family member as the main factor in their decision.

?Many people don?t think about these issues until it?s too late,? noted Brian Liu, Chairman of LegalZoom.com. ?The truth of the matter is that medical emergencies can happen at any time, without notice. A Maryland man detained in Aruba in the presumed death of his travel partner had an accidental-death insurance policy on her for $1.5 million and sought to claim the money two days after reporting her missing, a person who provided information to the investigation told The Associated Press. The details of the policy obtained by Gary Giordano before his trip to Aruba may help explain why he has been detained on suspicion of involvement in the death of Robyn Gardner, 35, of Frederick, during their short getaway to the Dutch Caribbean island. The person with knowledge of the policy told the AP that Giordano purchased a $1.5 million American Express Travel Insurance policy shortly before he left for Aruba and that the accidental-death benefit covered only the length of their short trip. The source told the AP that Giordano called the insurance company on Aug. 4, while the search for Gardner was still going on. He sought to confirm that the documents listing him as the insurance beneficiary had been received and wanted to begin redeeming the policy. This person also said that records indicate Giordano asked whether any search costs would be covered by the policy. The source did not have authorization to publicly release the information and agreed to speak with the AP only on condition of anonymity. A spokeswoman for American Express, Gail Wasserman, said the company could not comment on any individual policies for privacy reasons.Authorities also said they found explicit photos of Gardner on Giordano's camera. The nature of those photos is not known. Aruban Solicitor General Taco Stein, the spokesman for the investigation into Gardner's disappearance, said Wednesday that Giordano had travel insurance but he provided few details, saying investigators were still reviewing financial documents as they tried to determine what happened to the 35-year-old woman. Stein said investigators were still studying the policy to determine exactly what it entailed. "If you travel, you take out insurance. That's not an uncommon thing, to say the least," Stein told the AP. "It all depends on what's in the policy." Investigators were trying to determine if there was anything unusual or striking about the policy. "If you change the policy around and make it higher or whatever, then it may be of interest to the investigation but we haven't established that yet," he said. ABC news, citing an unidentified police source, first reported that Giordano had taken out a $1.5 million American Express insurance policy on Gardner and that it was being considered as a possible motive in her disappearance. Giordano's attorney, Michael Lopez, did not respond to a request to comment on the insurance policy. The attorney has previously said that there is no evidence that Giordano committed any crime and that he should be released. Giordano, a 50-year-old business owner from Gaithersburg, Maryland, traveled to Aruba with Gardner on July 31 and reported her missing two days later, saying she disappeared while the two were snorkeling. He initially assisted in the search but was detained at the airport as he tried to leave Aruba. Authorities said they had found discrepancies in his story. Giordano, a twice-divorced father of three sons, has denied any wrongdoing through his attorney. A judge ruled Monday there is enough evidence to hold him for at least 16 more days on suspicion of involvement in Gardner's presumed death. The woman's body has not been found and Aruban authorities on Thursday were preparing for a new, large-scale search of the island for her remains or other evidence. The FBI, which searched Giordano's home in an upscale Washington suburb, has been assisting Aruban authorities with the investigation and has put out a missing person poster with three photographs of the 5-foot-4-inch blonde, including one showing a jungle-print design tattoo on her left shoulder. The poster urges anyone with information about Gardner to contact the FBI or the nearest U.S. embassy or consulate. The exact nature of his relationship with Gardner, who had a boyfriend back home in Maryland, isn't clear but the prosecutor's office has said the couple stayed in a room together at a Marriott hotel in Aruba. Investigators said they have little information about what the couple did and how they behaved while on the island and have appealed for any witnesses to contact police.
